At Phoenix Rising we believe the real work begins when our clients leave residential treatment and enter back into their lives. We are committed to supporting our clients and their families as they move from sobriety to true recovery. We walk along side our clients and their families as they rebuild their lives, secure housing, seek employment, and repair relationships.

Phase 1 Treatment Liaison

Phoenix Rising provides ongoing case management while your family member is in treatment and includes the following components:

Collaboration with Treating Facility

Our clinical staff serves as a liaison between the family and treating facility. In this role we meet weekly with the residential treatment team and the family separately in an effort to address and communicate concerns, collaborate on the appropriate treatment plan, and receive updates on your loved one’s progress.

Family Systems Therapy & Support

We support the family through the entire process, from first call through discharge. While your loved one is in treatment we focus our efforts on helping the family system heal. It is important that upon discharge the client returns to a healthy environment. This service is specific to the family’s needs and includes weekly support calls with Phoenix Rising as well as individual and/or family therapy sessions with our licensed clinicians.

Preparation and Planning for Discharge

Establishing a strong discharge plan is critical to our client’s success. When it is time for your loved one to return from treatment, Phoenix Rising collaborates with the residential treatment team and the family to establish a safe and effective discharge plan. Comprehensive discharge plans put everyone at ease and provide a foundation for our clients and families to begin rebuilding their lives and relationships.

Phase 2 Rebuild and Recovery

Phoenix Rising recognizes that each client is uniquely designed, which means their recovery journey needs to be viewed through that same lens.

Rebuild & Recovery

We believe this is the most critical phase of our client’s recovery and where there is the most danger for relapse. It is very challenging to maintain sobriety and develop strong recovery outside the treatment facility, back in the “real world”. During this phase, Phoenix Rising supports the client and their family as they execute the discharge plan and develop a strong recovery program. During the “Rebuild and Recovery” phase of treatment we collaborate with the client to develop a “Master Needs List”, which documents and measures the clients own observable goals to include: housing, employment, school, physical/spiritual wellness, therapeutic healing, relational healing, and fun!
